Rules of participation in the prize game “Slogan write – diopter erase!”

• General information

1. The contest runs from 27.11 to 18.12.2014.The
2. winner will be announced no later than 25.12.2014.
3. The total prize fund is 200,000 dinars and includes:

1st prize: Preoperative examination and intervention of laser vision correction of both eyes (value 130,000 dinars)

2nd prize: Voucher with 50% discount to laser vision removal of both eyes (value 60,000 dinars)

3rd prize: Preoperative examination for laser vision correction (value 10,000 din.)

• Who is eligible?

1. Everyone over the age of 20 is eligible to participate.
2. The competition is of an international character, which means that persons who are not citizens of Serbia have the right to participate.
3. Prize winners should meet the following conditions:
   • Diopter stable for one year (that the diopter has not changed by more than +0.50/-0.50D during one year)
   • Diopter in the following ranges:
     • Farsightedness up to +5.0D
     • Myopia up to -10.0D
     • Astigmatism up to +/-5.0D
4. Participation is allowed by posting a slogan suggestion exclusively from one’s own (personal) Facebook profile.

• Dynamics of the competition

1. Participants should come up with a slogan as creative as possible on the topic of Laser Vision Correction.
2. Before setting up a slogan, you need to fill out an application form.

• first round of the contest involves voting in the form of “liking” the best slogan. Voting is limited by the duration of the prize game, and only fans of the Health House page have the right to vote. You can only vote for each slogan once.
• At the end of the voting period, the selection of 10 slogans with the highest number of “likes” is made, which go to the second round of the competition.
• In the second round of the competition, a jury composed of representatives of the “Perfect Vision” team will select the authors of the three most creative slogans, who will win the first, second and third prizes.

• At the end of the giveaway

1. The names of the lucky winners will be published on our official website perfectoriginal.update.care,Facebook page, and Twitter accounttag. All winners will also be notified individually via their Facebook accounts.
2. If the preoperative examination determines that the person who won the first prize is not a suitable candidate for laser vision correction, the second-placed participant automatically becomes the winner of the main prize. If the review determines that the second-placed participant is not a suitable candidate, the first prize is transferred to the third-placed participant, and so on.
3. In order for the previous rule to be successfully implemented, it is necessary for the winner of the first prize to report no later than 7 days from the date of the announcement of the results, and the other winners will be contacted later. If the winner does not respond within the specified period, they will not be entitled to use their prize.
4. During the collection of the prize, the lucky winners will be photographed with the staff of the Perfect Vision team. The organizer of the prize game reserves the right to publish these photos on the Internet, in order to transparently present the results of the prize game to a wider audience.
5. The organizer of the prize game reserves the right to use the selected slogans for promotional purposes (on printed and electronic material, outdoor campaigns, in radio and TV spots and in other forms of promotional activities).

• Responsibilities and Rights of the Organizer

1. Each participant is required to read the Rules of Participation. The organizer assumes the right to consider each participant to be familiar with the above rules.
2. The organizer is obliged to make sure that the voting is fair and correct, and to prevent any abuse that he observes.
3. The organizer undertakes that the slogans will not be misused or used for any purpose other than promotional.
4. The organizer undertakes not to use the photos of the lucky winners with the staff of the Perfect Vision team for any other purpose than to transparently present the results of the prize game to a wider audience.
5. The organizer is obliged to hand over the prizes to the winners within the agreed period and under the agreed conditions.
6. The organizer reserves the right to change these Rules, with the obligation to inform all participants in a timely manner by posting on the Facebook page.
