Diabetic retinopathy

Diabetes mellitus causes numerous vision problems such as: changes in refraction, elevated intraocular pressure, cataract. However, the most common condition is diabetic retinopathy, which occurs in more than 85% of people with diabetes and represents one of the most serious complications of diabetes.

How does diabetic retinopathy develop?

Diabetes damages the blood vessels in the eye, preventing proper blood supply and adequate nourishment of the retina. As a consequence of poor circulation in the eye due to long-standing and inadequately treated diabetes, new pathological blood vessels of poorer quality are formed, with thin, fragile walls that easily rupture and leak blood into the surrounding tissue.

Diabetic retinopathy

All of this results in the accumulation of fluid and blood in the area of the macula (the point of sharp vision) – diabetic macular edema, which leads to a decrease in vision. In the later stages of the disease, the patient loses vision due to bleeding into the vitreous body, and retinal detachment may also occur, as well as secondary glaucoma.

The diagnosis is made precisely and accurately by examining the fundus with dilated pupils, as well as with the help of OCT (optical coherence tomography). In the case of macular edema, it is sometimes necessary to perform fluorescein angiography (imaging of the fundus blood vessels using contrast). In the case of dense cataract and bleeding into the vitreous body – hemophthalmos, it is necessary to assess the condition of the retina by ultrasound.
